How do I put "4 Run pinned tests automatically, others manually" on the main VS toolbar, or give it a hot key?

I'm afraid that this is impossible. VS is quite precious about its toolbar elements, and adding a user controlled item to the toolbar dynamically is a challenge worthy of a project itself.

Note that the engine modes are accessible by key sequence - Try: Alt-U, S, 4
